چکیده
Learning Objectives: After studying this article, the participant should be able to: 1. Understand the indications for the use of free-tissue transfer in lower extremity reconstruction. 2. Understand modalities to enhance the healing and care of soft tissue and bone before free-tissue transfer. 3. Understand the lower extremity reconstructive ladder and the place of free-tissue transfer on the ladder. 4. Understand the specific principles of leg, foot, and ankle reconstruction. 5. Understand the factors that influence the decision to perform an immediate versus a delayed reconstruction.

Microsurgical Lower Extremity Reconstruction in the Subacute Period: A Safe Alternative
BACKGROUND Microsurgical reconstruction of the lower extremity is an integral part of the limb salvage algorithm. Success is defined by a pain-free functional extremity, with a healed fracture and sufficient durable soft tissue coverage. Although early flap coverage of lower extremity fractures is an important goal, it is not always feasible because of multiple factors. Between the years 2000 a...
متن کاملUsing a Y-shaped vein graft with drain-out branches to provide additional arterial sources for free flap reconstruction in injured lower extremities.
BACKGROUND Reconstruction of the distal lower extremity with poor arterial circulation is a challenge for reconstructive surgeons, especially when a microsurgical free tissue transfer is necessary. One may face the problem of having no suitable recipient artery to be used except the main one. To overcome this difficult situation, utilization of a Y-shaped vein graft (YVG) was developed. METHO...
متن کاملIs there any chance for microsurgical reconstructions in elderly?
In Italy there is a increase in the percentage of population aged over 70 with a gradual increase in average life expectancy (Table 1). Some studies show that septuagenarians have a greater chance of survival than octogenarians postoperatively and a lower incidence of mortality perioperatively [1]. Recent studies show that elderliness is not an independent risk factor for microsurgical complica...
متن کاملThe microsurgical revascularisation of resected segments of tibia in the dog.
A segment of tibia 4.5 centimetres long was removed from one hind limb of fifteen dogs. It was then replaced and the main vasculature was restored by a microsurgical technique. In eight controls the segment was replaced without such restoration. In two-thirds of the former cases the microvascular reconstruction was successful; the rate of infection was found to be reduced, bone union was guaran...
